All six weeks must be completed and totaled. Don't forget to total the grand total at the bottom and sign and date the document. 

Employer Evaluations - You are responsible for getting your employer to complete the attached form and return to your teacher by Sept. 28th at the start of class. 

Teacher Evaluation - First period teacher will complete the evaluation on your performace in thier class.
